PMID: 37963634; PMCID: PMC10644209.<\/p>\n<p><strong>Link to the article: <\/strong>https:\/\/pubmed.ncbi.nlm.nih.gov\/37963634\/<\/p>\n<p><strong>Summary: <\/strong><\/p>\n<p>Objective: The aim of this systematic review and meta-analysis is to provide an updated summary of the evidence on non-pharmacological interventions for children with autism, focusing on their effects on child development outcomes.<\/p>\n<p>Methods: An updated search was conducted in November 2021, including studies published between 2017 and 2021. Controlled studies, both experimental and quasi-experimental, were selected and grouped according to the type of intervention: behavioural, developmental, technology-based, animal-assisted, musical, sensory integration, TEACCH, and other sensory-related interventions. Outcomes were classified into domains such as social communication, adaptive behaviour, language, play, social-emotional behaviour, and diagnostic features of autism, among others.<\/p>\n<p>Results: 252 articles were included, with a total of 13,304 participants. In the meta-analysis, significant effects were found in randomised controlled trials for behavioural interventions on social-emotional behaviour (g=0.58; P=0.02) and developmental interventions on social communication (g=0.28; P=0.003). Developmental interventions also showed positive effects on social communication, and naturalistic developmental interventions showed positive effects on adaptive behaviour, language, play, and social communication. Technological interventions showed positive effects on social communication and challenging behaviour. However, adverse events were poorly monitored, although likely common.<\/p>\n<p>Conclusions: The study shows that there are interventions with significant evidence that improve certain outcomes in children with autism, although further research, especially of higher quality, is needed on some interventions.<\/p>\n<p>This scientific article has been classified as having <strong>a green light <\/strong> in terms of scientific evidence because it is a systematic review with meta-analysis. In this case, the study is an update of a previously conducted review, which ensures the incorporation of new relevant evidence and a more comprehensive analysis of the effects of interventions on outcomes in children with autism.<\/p>\n<p><strong>Methodological comments<\/strong><\/p>\n<p><a href=\"https:\/\/hacto.umh.es\/files\/2025\/02\/Verde-300x300-4.png\"><img loading=\"lazy\" class=\"wp-image-3316  alignleft\" src=\"https:\/\/hacto.umh.es\/files\/2025\/02\/Verde-300x300-4.png\" alt=\"\" width=\"234\" height=\"234\" srcset=\"https:\/\/hacto.umh.es\/files\/2025\/02\/Verde-300x300-4.png 300w, https:\/\/hacto.umh.es\/files\/2025\/02\/Verde-300x300-4-150x150.png 150w\" sizes=\"(max-width: 234px) 100vw, 234px\" \/><\/a>This study is classified as a systematic review due to several key characteristics described below (for more details on the different types of reviews, see this <a href=\"https:\/\/hacto.umh.es\/en\/2025\/02\/04\/tipos-de-revisiones-de-la-literatura-cientifica\/\">link<\/a>):<\/p>\n<ul>\n<li>It addresses a specific topic.<\/li>\n<li>It answers a specific question<\/li>\n<li>The study includes a systematic search in different databases.<\/li>\n<li>Clear and specific inclusion and exclusion criteria are included.<\/li>\n<li>A detailed evaluation of the methodological quality of the included studies is carried out, using recognised guidelines such as those of Cochrane to ensure the reliability of the results.<\/li>\n<li>A qualitative synthesis of the results is carried out..<\/li>\n<\/ul>\n<p>Furthermore, in addition to performing a quantitative synthesis of the results, the study includes a meta-analysis.<\/p>\n<p>Heterogeneity in a meta-analysis refers to the variability in the results of the included studies and can be measured using the \u03c4\u00b2 or I\u00b2 statistic. \u03c4\u00b2 quantifies the variability between effect sizes, while I\u00b2 indicates the proportion of the total variability attributable to heterogeneity rather than chance. In this study, the \u03c4\u00b2 statistic was used, and heterogeneity was considered high when the \u03c4\u00b2 value exceeded 0.5, which could raise concerns about the validity of combining the results of the studies. To combine the results, they used Hedges&#8217; g, a measure of effect size that indicates the magnitude of the difference between two groups in terms of standard deviations, adjusted to correct for bias in small sample sizes. Its interpretation varies according to the value (0: no difference; 0.2: small; 0.5: moderate; 0.8 or higher: large); the confidence interval (95% CI) indicates the range in which the true effect size is expected to lie with 95% certainty and is considered statistically significant if it does not include 0.<\/p>\n<p>For the critical appraisal of this article, we will follow the guidelines of the CASPe guidelines for systematic reviews (for more details, see the PublicaTO entry). We can also use the PRISMA guide (Preferred Reporting Items for Systematic Reviews), a tool from the EQUATOR network (for more information, see the PublicaTO entry ). This guide is used as a reference for writing systematic reviews. However, all authors declare that they have no other conflicts of interest. In our opinion, although the authors declare no other conflicts of interest, the receipt of fees or funding for activities related to interventions for children with ASD could influence their opinions or conclusions on the topic addressed in the study. We must carefully review the article, taking these potential biases into account, to critically assess whether the funding received may have influenced the interpretation of the results or the recommendations made, despite the authors&#8217; statements. This does not necessarily imply that the results are incorrect, but it does mean we must be aware of the possibility of bias when reading and analysing the study.<\/p>\n<p><strong>1) Was the review conducted on a clearly defined topic? <\/strong> Yes, although with some nuances. The review addresses a well-defined topic: non-pharmacological interventions aimed at improving developmental outcomes in children with ASD under 8 years of age. However, although the general topic is clearly defined, the study&#8217;s objective does not explicitly specify the specific types of intervention or the developmental outcomes that will be evaluated. This initial lack of precision may generate some ambiguity regarding the scope of the review. Nevertheless, the body of the article subsequently clarifies these elements by classifying the interventions (behavioural, developmental, natural, technological, etc.) and the domains assessed (social communication, language, adaptive behaviour, among others), which provides greater operational clarity to the analysis.<\/p>\n<p><strong>2) Did the authors search for the appropriate type of articles? <\/strong>Yes, the authors searched for the appropriate type of articles for their systematic review. They focused on experimental design studies, which are appropriate for evaluating the effectiveness of interventions in terms of developmental outcomes in children with ASD. The selected studies were:<\/p>\n<ol>\n<li>Randomised controlled trials (RCTs): These are the standard for evaluating the effectiveness of interventions, as they minimise bias and allow for the establishment of causal relationships.<\/li>\n<li>Quasi-experimental studies (or non-randomised trials, as we prefer to call them): These are appropriate when random assignment is not possible, which is common in interventions in clinical or educational settings.<\/li>\n<\/ol>\n<p>Furthermore, the search included both published and unpublished studies, ensuring broader coverage of the available evidence. Unpublished data were obtained through protocols registered with ClinicalTrials.gov and Journal Trials, which helps reduce publication bias. In this way, the authors followed an appropriate and comprehensive approach to obtain the most relevant and up-to-date data..<\/p>\n<p><strong>3) Do you think important and relevant studies were included? <\/strong>Overall, the authors included relevant and appropriate studies in their review, with some limitations regarding certain interventions such as animal-assisted therapy and cognitive-behavioural therapy, which could not be combined in the meta-analysis due to their small number. The authors mention that more research is needed in these areas.<\/p>\n<p><strong>4) Do you think the review authors made sufficient effort to assess the quality of the included studies? <\/strong> Yes, although with some nuances. The authors applied a rigorous methodological approach to assess the risk of bias, using recognised tools such as those proposed by Cochrane. They also used the heterogeneity index (\u03c4\u00b2) and robust estimation models to adjust the analyses, which improved the precision of the estimates despite inter-study variability. Furthermore, they performed subgroup analyses and a sub-analysis excluding low-quality studies, suggesting a conscious attempt to control for potential sources of bias and strengthen the internal validity of the findings.<\/p>\n<p>However, despite these efforts, the results do not entirely clarify how the bias assessment criteria were applied in each study. The figure presented to summarise this information is difficult to access and confusing, hindering a detailed assessment of the methodological quality of the included studies. In addition, the authors acknowledge that most studies did not adequately report adverse events. This represents a significant limitation when judging the safety of the interventions.<\/p>\n<p>Similarly, the specific tool used to assess the risk of bias is not clearly identified. It is also unclear whether any of the Cochrane Collaboration guidelines were used (e.g., RoB 2 for randomised trials or ROBINS-I for non-randomised studies). This lack of specificity limits the transparency of the assessment process. Furthermore, the figure summarising the bias assessment is unclear and difficult to interpret, hindering a thorough evaluation of the methodological quality of the included studies.<\/p>\n<p>In addition, the authors themselves acknowledge that many studies did not adequately monitor or report adverse events, which represents a significant limitation for assessing the effectiveness of the interventions analysed.<\/p>\n<p><strong>5) If the results of the different studies were combined to obtain a &#8220;pooled&#8221; result, was this reasonable? <\/strong>Yes, in this case, combining the results of the different studies to obtain a pooled result was reasonable, but with certain key considerations. The authors selected controlled studies (randomised and non-randomised clinical trials), which are appropriate for evaluating the effectiveness of the interventions and ensuring the quality of the pooled results. Nevertheless, it is important to note that the authors did not pool the non-randomised studies, which could be a limitation as it might exclude relevant information from non-randomised studies, which also provide valuable evidence, albeit with a higher risk of bias. Although heterogeneity was relatively high (\u03c4\u00b2 &gt; 0.5), the authors used a robust estimation model to handle the variability between studies, which allowed them to obtain more precise estimates despite the variability. Furthermore, they conducted subgroup analyses by intervention type and risk of bias, which helped clarify differences between outcomes and improved the internal validity of the meta-analysis. However, when heterogeneity exceeds 0.5, it is essential to investigate the cause of this variability before combining results, as it could indicate that the studies are too different from one another to be meaningfully combined. Therefore, while the overall combination of results was reasonable, it would be prudent to consider the high heterogeneity and explore its potential causes to ensure the accuracy and validity of the combined results.<\/p>\n<p><strong>6) What is the overall result of the review? <\/strong> The overall result of the review is that interventions for children with ASD show positive effects in areas such as social communication and emotional or challenging behaviour. Behavioural interventions and naturalistic-behavioural interventions have shown the most beneficial effects. However, the quality of the included studies has significant limitations, especially due to biases arising from self-reporting by caregivers or teachers and the lack of adequate monitoring of adverse events. In addition, the effects of the interventions were more pronounced in the specific areas directly addressed during the intervention, while changes in more general or long-term aspects were less significant.<\/p>\n<p><strong>7) What is the precision of the result(s)? <\/strong> The precision of the results in the study is reflected in the confidence intervals, which indicate the uncertainty about the actual effect of the interventions; for example, in behavioural interventions for social-emotional behaviour (g=0.58; 95% CI=0.11 to 1.06), the wide interval suggests that, although the effect is positive, there is considerable uncertainty about its actual magnitude.<\/p>\n<p><strong>8) Can the results be applied in your setting? <\/strong> Yes, the results could be applied in your setting, but taking into account cultural differences, resource limitations, available professional training, and local public policies. Adapting the interventions to the characteristics of the population, available resources, and cultural and economic realities is essential for successful implementation. Furthermore, collaboration among health professionals, educators, and families will be key to maximising the benefits of these interventions in the local context.<\/p>\n<p><strong>9) Have all important outcomes been considered in making the decision? <\/strong>Although the review addresses key outcomes such as improvements in social characteristics and behaviour, insufficient attention has been given to adverse events, long-term effects, and contextual and family factors, which are also important for making informed decisions about interventions.<\/p>\n<p><strong>10) Do the benefits outweigh the harms and costs? <\/strong> The benefits of interventions for children with autism, such as improved social communication and adaptive behaviour, are clear, especially with low-cost interventions like technology-based ones. However, concerns exist about unmonitored adverse effects, the high costs of intensive interventions, and the variability in study quality, making it necessary to consider these aspects when weighing the benefits against the harms and costs.<\/p>\n<p><strong>Relevance to clinical practice<\/strong><\/p>\n<ol>\n<li>Concept (main topic of the article)<\/li>\n<\/ol>\n<p>This article presents an updated systematic review and meta-analysis (Project AIM) of non-pharmacological interventions for children with ASD under 8 years of age. The quality and effectiveness of different types of interventions, such as behavioural, developmental, NDBI, and technology-based interventions, are evaluated in various areas of child development, including social communication, language, adaptive behaviour, and play. Furthermore, the study analyses the risk of bias in the reviewed studies, the temporal proximity of the interventions&#8217; effects, their generalisability outside the therapeutic setting and in other contexts, as well as the presence or absence of adverse or negative effects resulting from the therapies.<\/p>\n<ol start=\"2\">\n<li>Contextualization and Importance for Occupational Therapists<\/li>\n<\/ol>\n<p>This article is highly relevant for occupational therapists (OTs) because:<\/p>\n<ul>\n<li><strong>It provides updated evidence on<\/strong>\u00a0the effectiveness of the most commonly used approaches in early intervention for autism.<\/li>\n<li>It highlights the\u00a0<strong>methodological limitations <\/strong>\u00a0found in the research (such as the excessive use of measures based on caregiver reports, detection bias, lack of assessment of adverse effects), which may influence the interpretation of results and clinical practice.<\/li>\n<li>It emphasises the importance of considering approaches or models that provide <strong>generalisable and sustainable results <\/strong>\u00a0in natural environments, which aligns directly with occupation-centred practice and the everyday context, key principles of Occupational Therapy.<\/li>\n<li>It highlights, according to the criteria used in this research, the value of \u00a0<strong>natural and developmental<\/strong>\u00a0approaches such as NDBIs, which coincide with the ecological and relational framework of pediatric OT work.<\/li>\n<\/ul>\n<ol start=\"3\">\n<li>Key Messages<\/li>\n<\/ol>\n<ul>\n<li>Value NDBI and developmental interventions: NDBIs (e.g., Early Start Denver Model, JASPER, IMPACT) show positive effects on social communication, adaptive behaviour, and diagnostic characteristics of autism, even under rigorous criteria.<\/li>\n<\/ul>\n<p>Developmental interventions also improve social communication, although more objective measures are needed, according to the authors, to avoid bias.<\/p>\n<ul>\n<li>Avoid relying solely on parental reports: Much of the evidence has used caregivers&#8217; subjective perceptions as a measure. As OTs, it is crucial to complement this with \u00a0<strong>direct functional observation and objective, standardised assessment tools.<\/strong>.<\/li>\n<li>Include assessment and monitoring of adverse effects: Only 10% of the reviewed studies mention adverse events, and many do not actively monitor them. As OTs, we must include protocols that \u00a0<strong>detect signs of stress, distress, or unexpected consequences<\/strong>\u00a0for the child or family related to the methodologies we use.<\/li>\n<li>Encourage critical family participation: Parent-mediated interventions also require evaluating \u00a0<strong>their impact on parents. <\/strong>Examples cited in the article include \u00a0<strong>increased stress and negative effects on parental mental health <\/strong>\u00a0after implementing certain techniques. As the article points out:\u00a0<em>&#8220;Examples of adverse events that could be attributed to the intervention were intense child aggression and serious adverse effects on parent mental health after participating in a parent mediated intervention&#8221;<\/em>(BMJ, 2023, p.10).<\/li>\n<li>What about Sensory Integration in this systematic review? Sensory Integration Therapy remains a valid option, supported by positive initial evidence, but it has not yet reached the status of a \u201crobustly evidenced\u201d intervention according to strict systematic criteria such as those of Project AIM. Unlike behavioural interventions, most studies are quasi-experimental, case studies, or lack a control group. Its use should be based on individualised clinical assessment, fidelity to the model (ASI\u00ae), and prioritise functional and occupational outcomes, not just changes in sensory processing.
